For families navigating asthma, the Community Outreach for Asthma Care and Health (COACH) program offers added support at home, where it matters most. Available to families in Monmouth and Ocean counties enrolled in NJ FamilyCare/Medicaid or Charity Care, this program provides:
- In-home assessments by trained Community Health Workers
- Education on asthma medications and trigger management
- Personalized plans created with your child’s pediatrician and pulmonologist
Through a home visit, we help identify environmental triggers like dust, smoke, or pet dander that may be worsening your child’s symptoms. Research shows that managing these factors can reduce the need for medication and lead to more symptom-free days. Our goal is to improve your child’s comfort and your family’s peace of mind.
