Asthma Care at Home: The COACH Program

For families navigating asthma, the Community Outreach for Asthma Care and Health (COACH) program offers added support at home, where it matters most. Available to families in Monmouth and Ocean counties enrolled in NJ FamilyCare/Medicaid or Charity Care, this program provides:

  • In-home assessments by trained Community Health Workers
  • Education on asthma medications and trigger management
  • Personalized plans created with your child’s pediatrician and pulmonologist

Through a home visit, we help identify environmental triggers like dust, smoke, or pet dander that may be worsening your child’s symptoms. Research shows that managing these factors can reduce the need for medication and lead to more symptom-free days. Our goal is to improve your child’s comfort and your family’s peace of mind.

Helping You Navigate Your Child’s Lung Condition

Recurring coughing, wheezing, shortness of breath during play, or repeated lung infections may be signs of an underlying pulmonary condition. If these symptoms persist, it’s important to talk to your pediatrician or a pediatric pulmonologist.
Yes, with the right treatment plan, medication, and education about triggers, most children with asthma can lead active, healthy lives. Our team works closely with families to ensure asthma is well controlled.
If your child snores, gasps, or seems restless during sleep, a sleep study may be recommended. This test can help diagnose obstructive sleep apnea and other sleep-related breathing problems.
The in-home COACH program is currently limited to Monmouth and Ocean counties, but clinical care and education are available at multiple Hackensack Meridian Children’s Health locations statewide.
A typical visit may include a full health history, a physical exam, and breathing tests like spirometry. Your child’s provider will explain findings and create a care plan tailored to your child’s needs and your family’s goals.
